I highly doubt there is any insurance that covers personal trainers.

If you have a medical need for physical therapy … insurance may cover that, if ordered by your physician.

if your insurance specifically covers a personal trainer you need to contact the ins company, they will have list in your area that accept the ins

My medical insurance has never covered a personal trainer.

Some plans have health and wellness benefits. My plan covers half my gym membership fees. I think its possible there are plans that may cover personal trainers under health and wellness, the only way to find out is call your provider and ask.
